How to Enroll your Small Business
Thank you for selecting Healthcare Group as your preferred healthcare provider. After you have reviewed the available plans and rates, please follow these easy steps to enroll you and your eligible employees in a Healthcare Group benefit option plan.
Step 1: Download the enrollment materials and review the forms.
Step 2: Although all forms, benefit options, and premium rates are available to you in the enrollment packet and online, all newly-enrolling groups are encouraged to attend an enrollment workshop. The workshops will not only highlight the benefits of the plan options, but will explain the plan exclusions. It's important that you understand the eligibility requirements and the documents needed to enroll to ensure your desired effective date. Please call an enrollment specialist at 602.417.6755 (800.247.2289 outside Maricopa County) to schedule to attend an enrollment workshop (link) for additional information and assistance completing your enrollment forms.
Step 3: Complete the applicable forms in the enrollment packet.
Step 4: Return all required forms and your payment by the 15th of the month to ensure your effective date for the first of the following month.
You will receive your Member ID cards and handbook within 2-3 weeks of processing your enrollment paperwork.
