How to Enroll in HCTC

Once the notification of eligibility has been received by the HCTC Administration and you have received written notification of your eligibility, enrollment with HCG can begin. Click here if you have not received the eligibility notification or would like more information on the steps necessary to begin enrollment with HCTC.

Step 1: Download the HCTC enrollment packet and rates and review the forms.

Step 2: Benefit option availability is determined by your county of residence. Click here to review what is available in your county.

Step 3: Although all forms, benefit options, and premium rates are available to you in the enrollment packet and online, all newly-enrolling groups are encouraged to attend an enrollment workshop. The workshops will not only highlight the benefits of the plan options, but will explain the plan exclusions. It's important that you understand the documents needed to enroll to ensure your desired effective date. Please call an enrollment specialist at 602.417.6755 (800.247.2289 outside Maricopa County) to schedule to attend an enrollment workshop for additional information and assistance completing your enrollment forms.

Step 4: Complete and return all required forms with your payment by the 15th of the month to ensure your effective date for the first of the following month. Two month’s premium will be required at the beginning of enrollment and may be reimbursed through the HCTC Program. Click here for HCTC reimbursement request forms.

You will receive your Member ID cards and handbook within 2-3 weeks of processing your enrollment paperwork. The HCTC Program will send you an invoice once you are enrolled. You are then responsible for paying 20% of your health insurance premium to the HCTC Program by the due date each month. The HCTC Program then adds 80% to your payment and sends a check for 100% of your premium to your health plan.
